Based on the books and what we've seen so far in the show, here's what happens:

Belly and Jeremiah are together when Season 3 starts. They've been dating for two years, and things are getting super serious. Like, they get engaged and everything, which is a big shock to her family. But then, some stuff happens, and Belly finds out that Jeremiah cheated on her while they were "on a break" from their relationship. He feels really bad, so he proposes to her to show he's serious about her. She says yes, but it's not exactly a fairy tale, you know?

Meanwhile, Conrad has never really gotten over Belly. He's still got feelings for her, and when he's back in their lives, it makes things super complicated. At one point, he tells her he still loves her. This confession from Conrad leads to a big fight between the brothers on the day of Belly and Jeremiah's wedding. Jeremiah realizes that Belly's heart will always be with Conrad. He tells her that he won't be her "excuse" and that she needs to figure out what she wants on her own. So, they call off the wedding and break up.

After the breakup, Belly decides to focus on herself. She goes to college and even studies abroad in Spain. During this time, she and Conrad start writing to each other again, and they slowly get back in touch.

The books end with a time jump. We see Belly years later, at age 23. She and Conrad have reconnected, and they get married at Cousins Beach, which is where their whole story began. Jeremiah is even at the wedding, so it seems like the brothers have been able to mend their relationship, too.

So, in the end, Belly chooses Conrad. But it's not a simple choice, and she spends some time on her own before finally ending up with him. The show is following the books pretty closely, but there are a few changes, so we'll have to see exactly how it all plays out in the last few episodes.
